What is hospice care?

Hospice care focuses on comfort and quality of life, not curing, and is usually provided in the patient’s home. Hospice care is also provided in hospitals, nursing homes and assisted living facilities.  Services are available to patients of any age, religion, race, or illness. Hospice care is covered under Medicare, Medicaid, and most private insurance plans. Thomaston Hospice provides this care without regard to diagnosis, gender, sexual orientation, race, creed, disability, age, or ability to pay.

Who qualifies for hospice care?

Hospice care is for any person who has a life-limiting or terminal illness. Medicare, Medicaid, and Private Insurance require a prognosis of six months or less if the illness runs its normal course. All hospices consider the patient and family together as the unit of care.

How does hospice care work?

A family member or friend usually serves as the primary caregiver and, when necessary, helps make decisions for the terminally ill individual. Members of the Thomaston Hospice staff make regular visits to assess the patient and provide additional services. The hospice team develops a care plan that meets each patient’s individual needs for pain management, symptom control and custodial care. Support for caregivers is also assessed throughout the care period. The patient care plans vary depending on each individual and the specific needs, however a nurse at Thomaston Hospice is available by phone 24 hours a day/7 days a week.

What services are provided?

Thomaston Hospices’ interdisciplinary hospice team provides:

  • Nursing, Certified Nursing Assistant (CNA), Social Worker and Chaplain visits reflective of the individual patients Care Plan.
  • Patient’s pain and symptoms management to maintain comfort.
  • Provides needed drugs, medical supplies and DME equipment;
  • Educates the family/caregiver on how to care for their loved one;
  • Assists the patient and family with the emotional, psychosocial and spiritual aspects of dying;
  • Provides bereavement care to surviving loved ones.

Most individuals receiving hospice care are fully covered by the Medicare hospice benefit. Medicare covers virtually all aspects of hospice care. Most private health plans and Georgia Medicaid cover hospice services as well.
